Koepcke's Hermit is endemic to Peru where it is found in the forest understorey on the eastern slopes of the Andes at between 450 and 1,300 metres. See the distribution map at Birdlife International.

Distinguishing features are the long, straight bill, the tawny belly the whitish throat and the greyish whiskers.
