The Madagascar Fish-eagle is endemic to Madagascar where it is distributed in the west and north of the island. See the distribution map at Birdlife International. It is found mainly on the coast but can be found on larger rivers and lakes especially near woodlands.

It is classified as Critically Endangered given its low population of about 240 individuals and threats to its habitat.
